Patient Education / Safety Plan:
Patient encouraged to practice autonomy and utilize positive coping strategies, such as reaching out to a trusted friend when having a difficult day or needing to vent. For medication refills, patient should contact the pharmacy directly or schedule a follow-up visit if needed. If experiencing an unexpected medication side effect that was not previously discussed and no management plan is in place, patient should contact the office during business hours to schedule an appointment. For urgent medication side effects, such as rash or new involuntary muscle movements occurring outside of business hours, patient should go to the nearest emergency department.
If there is a sudden onset of psychosis, mania, or self-harm ideation that is not consistent with the patient’s usual pattern or severity, the patient should contact the office or present to the emergency department. If experiencing active suicidal ideation with a plan, or hearing new command hallucinations accompanied by persuasion to act or feelings of being unsafe, the patient should immediately go to the nearest emergency department.
Magnolia PMW is not equipped to provide emergency care or after-hours care. In the event of an emergency, the patient should call 911 or go to the local emergency room. For crisis support in the Triangle area, the patient may contact the Hopeline at 919-231-4525 or Wake Crisis and Assessment Services at 1-800-510-9132. The Suicide and Crisis Lifeline (988) is also available 24/7 for immediate assistance.

Patient encouraged to engage in self-care techniques, including:
• Identifying two supportive individuals to contact when in need
• Exercising 30 minutes per day
• Socializing and building healthy relationships
• Maintaining good nutrition and hydration
• Ensuring adequate sleep
• Practicing relaxation techniques
• Engaging consistently in psychotherapy
